OAKHURST – Four Kaiser Permanente physicians, who work at Kaiser’s Oakhurst Medical Offices, will be volunteering their time to give sports physicals to students at Yosemite High School on Thursday, May 23.
The physicians will provide the sports physicals to students who are planning to participate in athletic programs during the 2013-2014 school year. The physicals will begin at 5:30 p.m.Parents must complete and sign a medical history form prior to the physicals, which are available in the Yosemite High School office.
In addition to the medical history form, parents also will be asked to fill out a physical examination form with the name and birthdate of the student. Each physical costs $15, with all proceeds benefiting Yosemite High School.
